We collect your personal data when you voluntarily give it to us, in order to provide you with our independent nutrition and dietetic services. The data we collect includes name, address, email address, GP details, your child’s school details and any relevant medical information you share with us. We may also ask for further medical/clinical information in relation to reports from other health professionals if it is relevant to your consultation. This data is only collected with your consent. If you chose to send us emails, any personal information you disclose will also be stored on our email server.

Consent

The Children’s Nutrition Practice would like to keep your details for our records and we use these to help provide you with the most relevant nutritional advice. By using our website, you hereby consent to our privacy policy and agree to its terms.

What we do with the information we gather

We require this information for the purpose of:

  • Professional clinical record keeping of client information for 1:1 clients.
  • Sharing information with relevant health professionals, with your consent for 1:1 clients.

Security

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place encrypted electronic systems and standard operating procedures to safeguard and secure the information we collect.

Controlling your personal information

We will not distribute, sell or lease your personal information to third parties unless we have your explicit permission or are required by law to do so.
